Create a Program Logic Model

This workbook helps you create a simple logic model that identifies the key elements of your program that should lead to successful outcomes. Sections include:

  • Introduction
  • Design and Inputs
  • Outputs: Activities
  • Outputs: Participation
  • Outcomes
  • Additional Notes

When you submit your response to the workbook, you will get access to a PDF report that you can insert into funding proposals or communicate with stakeholders.

We created this tool to help organizations think about the most important parts of their program and what they need to evaluate. The best time to use it is when you are requesting funding. Many funders are now asking for logic models and evaluation plans as part of their proposals, and you may not have time or money to hire an evaluator at that point. This workbook, along with the associated Evaluation Planning Workbook, provides enough information for most funding proposals.

You can use it for as many programs as you wish. In fact, you don’t even need a SIS subscription. It was designed for a broad range of community services but you may find it helpful for other services as well.
